In consideration for receiving a free or reduced rate on a satellite TV system for the first time, new customers must activate the primary receiver with qualified programming (as defined by Promotion Guidelines) for 18 consecutive months from Dish Network.  The customer understands that in addition to the programming fee, state and local takes apply to all equipment and installation.  The customer upon execution of this form, acknowledges that in the event of default (seasonal or hard disconnect, payment delinquency, and/or downgrading from qualified programming package, etc.), I authorize The Sound Wave Electronics to charge the Promotion Discount (listed on invoice) to the credit card listed above.  If for some reason the credit card company declines the charge, I agree to submit full payment within 30 days of written notification.

Equipment warranties vary with each manufacturer and is detailed in the paperwork with each piece of equipment.  Installation is warranted for 90 days from the date of installation.  All labor charges are non-refundable.

We could like for you to be able to enjoy your satellite equipment for a long time, which may not be possible without good surge protection.  Be aware of the dangers of not having a surge protector connected to your satellite TV system, TV and other home electronic components.  Power surges do terrible damage and we want your equipment to be safe.  The surge protector we offer is a Powermax 8 DBS+3, which provides a lifetime $100,000 connected equipment warranty.  We offer this because we feel it provides the best protection.  This surge protector provides enough plugs for your satellite equipment, phone line and other electronics.  It also includes diagnostic lights for your safety.  The cost is $9999 for one or $79.99 each for two or more which includes installation at the time of satellite installation.  Your installation technician will be happy to install one for you.
